What Is MIS Reporting?
The DOT MIS Report is an annual summary of a company's drug and alcohol testing activities. It is submitted to the **Office of Drug and Alcohol Policy and Compliance (ODAPC) or the appropriate DOT agency (e.g., FMCSA, FAA, FTA, PHMSA, FRA, USCG) upon request. Most DOT agencies require all MIS Reports to be submitted by March 15th for the previous year's data.
MIS reports provide detailed information about:
- The number of employees in safety-sensitive roles
- The number and types of drug and alcohol tests conducted
- Test results (e.g., positive, negative, refusals)
- Follow-up testing data

Who Is Required to Submit a DOT MIS Report?
Not every DOT-regulated employer is required to submit a report every year. However, you must comply if:
- You are **notified by a DOT agency** (such as FMCSA) that your company has been selected to report.
- You are a large employer or part of a **random selection sample** from a DOT agency.
- You have safety-sensitive employees and conduct **DOT drug and alcohol testing**.

What Information Is Included in the MIS Report?
A standard DOT MIS report includes:

You must **retain testing records for five years** and be prepared to provide them to DOT upon request—even if you're not selected for MIS reporting that year.
