Dear Valued Client/Consortium Member,

Thank you for updating your random pool/consortium list. Please find below instructions on how to download the required form, enter the information and submit the updated information directly to our Randoms Department.

Instructions for updating your enrollment list:

1. Download an example of our enrollment list below. (please do not alter the file in any manner)

2. Enter the required information for ALL ACTIVE employees ONLY

3. Save the excel file

4. Complete the Required information below and submit the excel file

Please Note: Only employees listed on the uploaded excel sheet will be enrolled. Any employees not listed will be removed from random drug & alcohol testing pool. Also, the list you submit will replace all previous enrollment lists.
