A sample of 1.5 inches consisting of around 200 hairs (roughly the size of a #2 pencil) closest to the scalp provides 100mg, which is optimal for screening and confirming. For tests such as EtG or those over 10 panels, 150mg is suggested. We advise using a jeweler’s scale to weigh the specimen. If scalp hair isn’t available, a comparable quantity of body hair is acceptable. "Head hair" refers exclusively to scalp hair. All other hair types, including facial and axillary, are classified as "body hair."

Process Overview

The laboratory processing of drug test results is broken down into four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary processing of a sample into the lab's system. This step ensures the sample's seal is intact, it was shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and concludes any further data entry absent in an electronic chain of custody system.

Screening acts as an initial, swift check for drug misuse. Though Screening offers a cost-conscious method to disqualify drug use for most samples, a positive result needs confirmation for court validity. Samples preliminarily positive in Screening require secondary confirmation.

If Screening yields a presumptive positive result, more hair is taken from the initial sample for Extraction. During this phase, drugs are isolated from hair at a far reduced concentration compared to other methods (e.g., urine or oral fluid), explaining why hair drug screening is particularly challenging.

Positive screenings are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as necessary. Every laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
