What it is, How it works

Hair captures a prolonged record of alcohol and other substances by embedding biomarkers within the strands as they grow. When collected near the scalp, hair can offer an approximate 3-month detection window for alcohol and drugs. It is straightforward to gather, challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches, consisting of around 200 hair strands (comparable to the diameter of a #2 pencil) nearest to the scalp, provides 100mg of hair, which is the preferred amount for both screening and confirming. For EtG, special add-ons, and/or tests surpassing 10 panels, 150mg of the sample is advocated. We advise weighing the sample on a jeweler’s scale. In the event scalp hair is not accessible, an equivalent volume of body hair can be collected. When mentioning head hair, we pertain solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four principal stages in the laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial handling of a sample within a laboratory’s framework. This process involves checking that the sample was properly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry absent in an electronic chain of custody system.

Screening encompasses an initial swift examination for abuse drugs. While Screening serves as a cost-efficient measure to exclude drug usage in the bulk of samples, a positive screen necessitates confirmation to be eligible in legal proceedings. Any samples that demonstrate a presumptively positive result in Screening must undergo secondary confirmation.

In the event a sample reveals a presumptively positive result in the Screening phase, further hair is extracted from the initial specimen and set for Extraction. In this phase, drugs are isolated from hair at a significantly lower concentration than in alternative methodologies (e.g., urine or oral fluid), which accounts for the difficulty in conducting hair drug screening.

Confirmation of any positive screening findings is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are also washed prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
