What it is, How it works

Hair testing is now widely regarded as a robust method for identifying drug and alcohol consumption. Hair captures a comprehensive long-term record of alcohol and drug use by embedding biomarkers within the fibers of the growing hair strand. When sampled near the scalp, hair can offer a detection timeframe of approximately 3 months for both alcohol and drugs. The collection process is straightforward, not easily tampered with, and convenient for transportation.

A sample measuring 1.5 inches with around 200 strands of hair (comparable to the thickness of a #2 pencil) taken nearest the scalp provides 100mg of hair, which is the optimal amount for preliminary and confirmatory testing. For additional tests such as EtG or those exceeding 10 panels, a specimen weight of 150mg is suggested. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is not available, a similar quantity of body hair may be used. The term head hair specifically refers to scalp hair, whereas body hair encompasses all other forms (facial, axillary, etc.).

Process Overview

There are four fundamental stages in processing a drug test result in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into the lab's system. This stage includes ensuring that the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and undertaking any additional data entry not integrated by an electronic chain of custody system.

Screening carries out an initial rapid assessment to identify drug presence. While this is a cost-efficient method to eliminate drug use in the majority of samples, any positive findings in Screening must be validated for court admissibility. Samples testing presumptively positive in Screening require further confirmation.

Should a sample test presumptively positive at the Screening phase, additional hair is prepared from the initial sample for Extraction. In this phase, drugs are separated from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening presents significant challenges.

Any positive results from screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation if necessary. The comprehensive laboratory process, from Accessioning to Confirmation, is subject to review in accordance with the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 certification standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
