What it is, How it works

Hair analysis has emerged as an effective method for identifying drug and alcohol consumption. Hair holds a prolonged record of alcohol and drug presence by embedding biomarkers in its fibers as it grows. When cut near the scalp, hair offers a detection period of roughly up to 3 months for alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and easy to send.

A sample of 1.5 inches, consisting of about 200 hair strands (similar in thickness to a #2 pencil) from near the scalp, yields 100mg of hair, the preferable quantity for both screening and confirmation. For EtG, additional tests, and tests beyond 10 panels, it is advised to collect 150mg of the hair sample. We suggest using a jeweler’s scale for weighing the sample. When scalp hair is not available, an equivalent amount of body hair can be obtained. "Head hair" specifically refers to scalp hair alone, whereas "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four basic steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage of integrating a sample into the laboratory system. It ensures the sample's sealing and shipment were proper, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry that an electronic chain of custody might not provide.

Screening is a preliminary rapid check for potential drug abuse. While cost-efficient for eliminating drug use in most cases, a positive result in Screening requires confirmation for legal proceedings. Any samples indicated as presumptively positive in Screening necessitate a further confirmation step.

In the event of a presumptive positive during the Screening phase, additional hair is extracted from the initial sample and prepped for the Extraction step. In this phase, the extraction of drugs from the hair occurs at concentrations much lower than in other methods (e.g., urine or oral fluid), making hair drug testing a particularly complex methodology.

Confirmation of any positive screening results is executed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All presumptive positive samples undergo cleaning as necessary prior to confirmation. The complete lab process from Accessioning through Confirmation is examined adhering to both the CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
