A sample that measures 1.5 inches and contains around 200 hair strands (roughly the size of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the ideal quantity for both screening and confirmation. For EtG testing, additional panels, or tests greater than 10 panels, 150mg of the hair sample is advised. Using a jeweler’s scale to weigh the sample is suggested. Should scalp hair be unavailable, a similar quantity of body hair can be used. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Overview of the Process

There are four central steps in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

The initial processing of a sample into a laboratory system occurs during Accessioning. This step confirms that the sample was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and fulfills any data entry tasks that were not completed by an electronic chain of custody system.

Screening involves a preliminary quick examination for drugs of abuse. Although Screening is a cost-effective measure to exclude drug usage in the majority of samples, a positive screen must be confirmed for it to be permissible in court. Any samples that appear positive during Screening require a subsequent confirmation.

If a sample appears positive during the Screening phase, additional hair is taken from the initial sample and set aside for Extraction. At this stage, drugs are extracted from hair at a lower concentration level than in other methods (for instance, urine or oral fluid), which is why hair drug screening is considered the most challenging method to execute.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that appear positive are washed prior to confirmation as necessary. The entire laboratory sequence from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation as well as the accreditation to ISO / IEC 17025 standards.
