A 1.5-inch section comprising approximately 200 hair strands (similar in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for analysis and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest weighing the specimen using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be collected. When mentioning head hair, we are referring specifically to scalp hair. Body hair encompasses all other hair kinds (facial, axillary, etc.).

Process Overview

The primary steps in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample into a laboratory’s system. This includes confirming that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not provided by an electronic chain of custody system.

Screening entails an initial rapid check for drugs of abuse. While Screening is a cost-efficient way to rule out drug usage in most samples, a positive screen must be verified to be acceptable in court. Any samples that are presumptively positive in Screening do necessitate a secondary confirmation.

If a sample tests presumptively positive in the Screening phase, more hair is taken from the original specimen and prepared for Extraction. During this phase, drugs are extracted from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex method to execute.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are washed prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation according to ISO / IEC 17025 standards.
