Should scalp hair be unavailable, an equivalent volume of body hair can suffice. When mentioning head hair, it specifically means scalp hair, whereas body hair pertains to other types (facial, axillary, etc.).

Process Overview

The procedure for analyzing a drug test result in the laboratory includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample within a lab's system. This involves verifying that the sample is correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not provided by an electronic chain of custody system.

Screening offers an initial rapid assessment for drugs of concern. Although Screening is an economical approach to dismiss drug use in most cases, a positive result must be validated to be court-admissible. Any samples that test positive in Screening require further confirmation.

If a sample tests positive during Screening, additional hair is extracted from the original sample and processed for Extraction. During this phase, substances are isolated from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), highlighting why hair drug testing is the most challenging to conduct.

Verification of any positive screening result utilizes techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. Prior to final validation, all presumptive positives undergo a washing process. The entire laboratory operation, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
