What it is, How it works

Hair offers an extended historical record of substances by embedding biomarkers within the strands during growth. When obtained from the scalp, hair can reveal substance use for up to three months. Collecting hair is straightforward, somewhat resistant to tampering, and easy to transport.

A 1.5-inch segment of roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp provides 100mg, which is ideal for screening and verifying. For EtG, additional tests, or panels exceeding 10, a sample of 150mg is preferred. It's advisable to measure the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be used. When discussing head hair, we mean only scalp hair. Body hair includes all other hair types (e.g., facial, axillary).

Process Overview

The lab processing of a drug test encompasses four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where a sample is introduced into the lab’s system. This step entails verifying proper sealing and shipping of the sample, assigning it a random LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody.

Screening is a preliminary evaluation looking for abusive substances. Although a cost-efficient way to exclude drug use in most samples, a positive result in Screening must be confirmed for courtroom acceptance. Presumptively positive samples in Screening require further confirmation.

If Screening indicates a presumptive positive, additional hair is taken from the main sample for Extraction. Here, drugs are extracted from the hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging to administer.

Confirmation of positive results from Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation if needed. The entire lab process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
