What it is, How it works

Hair testing is now recognized as a robust method for identifying drug and alcohol consumption. Hair effectively captures biomarkers within its fibers, providing a comprehensive, long-term record of substance use. When sampled near the scalp, hair can offer a detection time frame of up to about three months for both alcohol and drugs. The collection process for hair is straightforward, resistant to tampering, and shipping is uncomplicated.

Taking a 1.5-inch segment, comprising roughly 200 hair strands (similar in diameter to a #2 pencil) from close to the scalp will yield 100mg of hair, which is the optimal quantity for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen weight of 150mg is advised. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair is not an option, a comparable quantity of body hair can be collected. When we mention head hair, we refer exclusively to scalp hair, whereas body hair encompasses all other varieties (e.g., facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four pivotal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage, where a sample is integrated into a laboratory's system. This step involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry that an electronic chain of custody system does not provide.

Screening provides a preliminary assessment for substances of abuse. Though an economical approach to ruling out drug use in most samples, any positive screening result must be validated to hold up in legal settings. Samples that indicate a presumptive positive in Screening need an additional confirmation step.

Should Screening suggest a presumptive positive, additional hair is sourced from the original sample for Preparation for Extraction. During this stage, substances are extracted at a notably lower concentration from hair compared to other methods (such as urine or oral fluid), highlighting the complexity of performing hair drug screenings.

Confirmation is executed via GC/MS, GC/MS/MS, or LC/MS/MS for any positive screening results. Prior to confirmation, all presumptive positive samples undergo washing when necessary. The entire laboratory process, from Accessioning through to Confirmation, is conducted under the oversight of both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
