What it is, How it works

Hair analysis has emerged as an effective means for identifying the consumption of drugs and alcohol. Hair captures a prolonged record of alcohol and drug usage by entangling biomarkers within the fibers of the hair as it grows. When acquired near the scalp, hair can reveal a detection timeframe of nearly three months for both alcohol and drugs. The collection process of hair is straightforward, relatively challenging to tamper with, and uncomplicated to transport.

A hair sample measuring 1.5 inches, consisting of roughly 200 strands (comparable to the width of a #2 pencil) nearest to the scalp, will yield 100mg of hair, which serves as the optimal specimen for both screening and confirmation. For EtG, additional analyses, and/or examinations exceeding 10 panels, it is advisable to gather 150mg of the sample. We suggest utilizing a jeweler’s scale for weighing the sample. Should scalp hair be inaccessible, an equivalent quantity of body hair can be collected. When we allude to head hair, we specifically mean scalp hair. Body hair denotes all other varieties of hair (e.g., facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is comprised of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample within a laboratory framework. This encompasses verifying that the sample was correctly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and finalizing any supplemental data entry absent from an electronic chain of custody system.

Screening entails a preliminary rapid evaluation for drugs of abuse. Although Screening serves as an economical approach to exclude drug consumption in the majority of samples, a positive screen mandates confirmation to be permissible in legal proceedings. All samples appearing presumptively positive in Screening necessitate a subsequent confirmation.

Should a sample present as presumptively positive during the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. At this juncture, drugs are extracted from hair at notably lower concentrations than in alternative methodologies (e.g., urine or oral fluid), thus making hair drug screening the most complex methodology to execute.

Validation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undertake washing prior to confirmation, if necessary. The complete laboratory sequence from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and in accordance with the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection timeframe: Hair drug tests can identify drug use up to 90 days in the past, whereas urine tests offer a shorter detection period.
  • Hard to manipulate: Hair drug tests are exceptionally difficult to falsify, leading to more precise results.
  • Delivers historical insight: It can reveal a trend of drug consumption over an extended timeframe, not merely recent usage.

Limitations:

  • Incapable of identifying recent consumption: Drugs require around 5-7 days to become detectable in hair.
  • Expense: Hair drug tests are typically more costly compared to other drug testing techniques.
  • Inconsistencies in outcomes: Factors such as hair color and unique variations in hair growth can influence the concentration of drug metabolites within the hair.

Note: Although frequently called "hair follicle tests", this examination evaluates the hair strand rather than the follicle beneath the scalp
