What it is, How it works

A 1.5-inch section of around 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is ideal for screening and confirmation. For EtG, additional tests, and/or more than 10-panel tests, 150mg is advised. We suggest weighing the sample with a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be used. When we mention head hair, we're specifically talking about scalp hair. All other hair types (facial, axillary, etc.) are considered body hair.

Process Overview

The four essential phases in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial entry of a sample into lab systems, including checking if the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and filling in any missing data not supplied by an electronic chain of custody system.

Screening is a preliminary check for drugs of abuse. It's a cost-effective approach to identifying drug use in most samples. However, a positive result requires confirmation to be valid in legal contexts. Samples that test presumptively positive during Screening must be confirmed through additional testing.

When a sample tests presumptively positive at the Screening stage, additional hair is taken for Extraction. During Extraction, drugs are retrieved from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Confirmation of positive Screening results occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as necessary. The full process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
