For EtG, additional procedures, and/or assessments exceeding 10 panels, 150mg of the sample is advised. It is suggested to measure the sample with a jeweler’s scale. If scalp hair is not obtainable, an equivalent amount of body hair may be gathered. When talking about head hair, it specifically refers to scalp hair. Body hair includes all other hair types (including facial, axillary, etc.).

Process Overview

Accessioning entails the preliminary processing of a specimen into the laboratory’s records. This process ensures the sample was sealed and transported adequately, assigns a random LAN (Laboratory Accessioning Number), and handles any extra data entry not covered by the electronic custody chain system.

Screening executes an initial brief evaluation for drug abuse. Although Screening is a cost-efficient method to dismiss drug usage on the bulk of samples, a positive outcome requires confirmation for legal acceptance. Any specimens that yield a presumptive positive in Screening necessitate further confirmation.

If a specimen is reportedly positive in the Screening stage, a larger portion of hair is extracted from the original sample and readied for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than other methods (like urine or saliva), which explains why hair drug analysis is the most challenging procedure to conduct.

Confirmation for any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation when required. The complete laboratory procedure from Accessioning to Confirmation is monitored under both the CAP (College of American Pathologists) Hair classification and accreditation to ISO / IEC 17025 standards.

Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
