What it is, How it works

Hair strands capture a long-term record of alcohol and drug presence by embedding biomarkers within the fibers of the hair as it grows. When hair is obtained near the scalp, it can offer approximately a 3-month detection period for substances like alcohol and other drugs. Hair collection is straightforward, relatively resistant to tampering, and simple to transport.

A 1.5-inch section comprising around 200 strands of hair (approximately the width of a #2 pencil) nearest to the scalp generates 100mg of hair, which is the optimal quantity for both screening and confirmation processes. For EtG add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. Using a jeweler’s scale to measure the specimen is recommended. If scalp hair is not available, an equivalent amount of body hair can be gathered. References to head hair solely pertain to scalp hair, while body hair includes all other hair varieties (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result consists of four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the laboratory system, which involves verifying the sample's proper sealing and shipment, assigning a random Laboratory Accessioning Number (LAN), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening involves a rapid preliminary check for drug abuse signs. While it is a cost-efficient method to exclude drug use for most samples, any positive screening result requires confirmation to be valid in legal settings. Samples that presumptively test positive during this stage need secondary confirmation.

Should a sample test presumptively positive at the Screening stage, additional hair is taken from the initial sample and prepped for Extraction. In this stage, drugs are withdrawn from hair at significantly lower concentrations compared to other testing mediums (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of positive screening results is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptively positive receive a pre-confirmation wash if necessary. The entire lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation alongside the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
