Recently, hair follicle testing has gained traction as a drug testing method. Employers, courts, and Substance Abuse professionals now prefer it over traditional urine tests. Employers with zero-tolerance policies, judicial systems, and probationary programs favor hair tests for their extended detection window of up to 90 days, collected under observation, making them nearly foolproof. However, for usage within the past 5 days, urine testing remains more precise.

What it is, How it works

Hair analysis has become recognized as an effective means for identifying drug and alcohol consumption. Hair captures a comprehensive timeline of alcohol and drug usage by securing biomarkers in the fibers of the developing hair strand. When obtained near the scalp, hair can offer an approximately 3-month detection period for alcohol and other drugs. Collecting hair is straightforward, relatively resistant to tampering, and convenient to transport.

A 1.5-inch portion of roughly 200 hair strands (comparable to the width of a #2 pencil) near the scalp will provide 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional tests, and/or examinations exceeding 10 panels, it is recommended to acquire 150mg of the sample. We advise measuring the sample with a jeweler's scale. If scalp hair is not present, an equivalent quantity of body hair can be collected. By head hair, we mean only scalp hair. Body hair encompasses all other types (facial, underarm, etc.).

Process Overview

The key procedures involved in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into the lab's system. This covers verifying proper sealing and shipping of the sample, assigning it a random LAN (Laboratory Accessioning Number), and completing any further data entry not included in an electronic custody chain.

Screening signifies a preliminary assessment for potential drug use. Though Screening is cost-efficient for dismissing drug use in the majority of samples, a positive screening must be validated for legal proceedings. Samples that are presumptively positive in Screening require a second confirmation.

If a sample indicates a presumptive positive during Screening, additional hair is extracted from the primary specimen and prepared for Extraction. Here, drugs are retrieved from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing one of the most challenging methodologies.

Confirmation of any positive screening is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is assessed under both CAP (College of American Pathologists) Hair designation and accreditation in compliance with ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
