For EtG add-ons and/or more comprehensive tests exceeding 10 panels, a sample with a minimum of 150mg is suggested. Utilizing a jeweler’s scale to weigh the hair specimen is advised. If scalp hair is not available, an equivalent volume of body hair may be utilized. By scalp hair, we mean head hair alone. Body hair describes all other types of hair (such as facial and axillary hair).

Process Overview

The laboratory process of determining a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of the sample into a laboratory's system. The process confirms that the sample was appropriately sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and concludes any extra data entry absent from an electronic chain of custody system.

Screening includes a preliminary check for illicit substances. Although Screening is an effective, low-cost method to rule out drug use on most samples, any positive screen must be confirmed to be valid in legal settings. Any samples that come back presumptively positive during Screening must undergo secondary confirmation.

If a sample shows presumptive positivity during Screening, additional hair is obtained from the existing specimen and readied for Extraction. In this stage, substances are extracted from the hair in significantly lower concentrations than in other testing methods (such as urine or oral fluid), which highlights the complexity of hair drug screening.

To verify any positive screening result, GC/MS, GC/MS/MS or LC/MS/MS methods are utilized. All presumptive positive samples are cleaned prior to confirmation when necessary. Each part of the laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
