For testing, a 1.5-inch segment consisting of around 200 hair strands (roughly the diameter of a #2 pencil), gathered nearest the scalp, yields 100mg, the optimal sample size for initial and follow-up tests. For EtG, additional tests, and/or evaluations beyond 10 panels, a 150mg sample is advised. We suggest assessing the specimen's weight using a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may be utilized. Head hair refers exclusively to scalp hair, while body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a lab's system. This step verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for drug presence. Although Screening serves as an economical method to exclude drug use in the bulk of samples, a positive screen necessitates confirmation for legal admissibility. Samples that are provisionally positive in Screening require further confirmation.

Should a sample test positive in the Screening stage, more hair is extracted from the initial sample and prepped for Extraction. In this phase, drugs are isolated from hair at lower concentrations compared to other methods (such as urine or oral fluid), which is why hair drug assessment is a challenging methodology.

Confirmation of any positive screening involves techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are cleansed prior to confirmation as required. The comprehensive process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
