For EtG, add-ons, or tests exceeding 10 panels, a 150mg sample is advised. It's recommended to assess the specimen's weight using a jeweler’s scale. In the absence of scalp hair, an equivalent amount of body hair can be taken. When discussing head hair, we mean scalp hair specifically. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial intake of a sample into a laboratory’s workflow. This stage involves checking that the sample is sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and performing any necessary data entry not covered by an electronic chain of custody.

Screening provides an initial rapid assessment for drug substances. Although Screening is a cost-efficient method for negating drug presence in most samples, a positive screen requires confirmation to be valid in legal contexts. Samples that produce a presumptive positive in Screening mandate a follow-up confirmation test.

If presumptively positive at Screening, additional hair is extracted from the primary specimen for the Extraction phase. Here, drugs are isolated from hair at much lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Positive screening outcomes are verified through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with initial positive results are washed before confirmation if necessary. The end-to-end laboratory procedure from Accessioning to Confirmation undergoes review in line with the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
