The collection process is straightforward, challenging to tamper with, and easy to transport.

A sample measuring 1.5 inches with about 200 hair strands (comparable to the thickness of a #2 pencil) closest to the scalp yields 100mg, an ideal quantity for testing. For EtG and tests exceeding 10 panels, a specimen of 150mg is advisable. Using a calibrated scale to weigh the specimen is suggested. If scalp hair cannot be sourced, an equivalent amount of body hair is acceptable. 'Head hair' refers exclusively to scalp hair, while 'body hair' encompasses other types of hair (such as facial or axillary).

Process Overview

The laboratory processing of a drug test result involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the laboratory's system. This step ensures that the sample was appropriately sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data input not captured by an electronic chain of custody system.

The Screening phase provides a preliminary review for substances of abuse. Though Screening serves as a cost-efficient means to eliminate drug use for most samples, any positive screening test requires confirmation to be court-admissible. Samples suggesting a positive result during Screening necessitate further verification.

Should a sample appear positive after Screening, additional hair is sourced from the original collection for Extraction. At this stage, drug elements are extracted from hair at significantly lower levels compared to other methods (e.g., urine or saliva), affirming that hair drug testing is the most challenging process to conduct.

Confirmation for any positive screening outcome is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as necessary. The complete lab process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
