What it is, How it works

When acquired near the scalp, hair can provide a detection period for alcohol and other substances of up to roughly three months. Hair is straightforward to gather, relatively hard to alter, and simple to transport.

A 1.5-inch portion consisting of around 200 hair strands (approximately equivalent in size to a #2 pencil) nearest to the scalp offers 100mg of hair, the ideal specimen quantity for both screening and validation. For EtG, additional tests, and/or panels exceeding ten, a sample of 150mg is advised. We suggest using a jeweler’s scale to weigh the specimen. Should scalp hair be absent, a similar amount of body hair may be collected. When referring to head hair, we denote this as scalp hair exclusively. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory procedure for drug test results includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial integration of a sample into a laboratory’s framework. This encompasses confirming that the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not available through an electronic chain of custody system.

Screening represents an initial rapid inspection for substance abuse. Although Screening is an economical way to exclude drug usage for most samples, a positive screen requires confirmation to be valid in legal proceedings. Any samples with presumptive positives in Screening necessitate a secondary validation.

If a sample is potentially positive during the Screening phase, additional hair is retrieved from the original specimen and prepared for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging technique to execute.

Verification of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation as necessary. The entire laboratory workflow from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
