Hair collected near the scalp can reveal drug and alcohol use for up to three months. It is straightforward to collect, relatively hard to tamper with, and convenient to transport.

A hair sample measuring 1.5 inches with approximately 200 strands (roughly the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair—the optimal amount for initial and confirmatory analysis. For EtG, additional tests, or panels beyond 10, 150mg of hair is suggested. Using a jeweler's scale to weigh the sample is advisable. Should scalp hair be inaccessible, a similar mass of body hair can be utilized. We use "head hair" to denote scalp hair specifically, while "body hair" encompasses any other type (such as facial or axillary).

Process Overview

The laboratory drug testing process includes four key stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is logged into the lab's system. This step ensures the sample was properly sealed and sent, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any necessary data entry absent from the electronic chain of custody.

Screening is the preliminary check for substances of abuse. Although Screening efficiently and economically rules out drug use in most samples, a positive result requires further confirmation for legal validity. Samples that test positively during Screening must undergo confirmatory testing.

When a sample tests positive in Screening, more hair is extracted from the original specimen for the Extraction phase. In this step, drugs are isolated from hair at a lower concentration compared to other methods like urine or oral fluids, accounting for the complexity of hair drug analysis.

Confirmation of positive Screening results is carried out through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to Confirmation, all presumptive positive samples are washed if necessary. The entire lab process, from Accessioning to Confirmation, is assessed under the CAP (College of American Pathologists) Hair designation and complies with ISO/IEC 17025 standards.
