What it is, How it works

Hair analysis is now regarded as an effective means for identifying drug and alcohol consumption. Hair captures a historical profile of alcohol and other substances by embedding biomarkers within the strands of the growing hair. When gathered near the scalp, hair can offer up to around a three-month timeframe for identifying alcohol and other substances. Hair is straightforward to gather, relatively hard to tamper with, and simple to transport.

A 1.5-inch section consisting of about 200 hairs (roughly matching the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, the perfect quantity for analysis and verification. For EtG, additional tests, and/or evaluations over 10 panels, 150mg of the sample is suggested. We advise determining the weight of the sample using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be gathered. When mentioning head hair, only scalp hair is implied. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary phases involved in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a specimen in a laboratory’s setup. This entails ensuring that the specimen was correctly secured and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not supplied by an electronic chain of custody arrangement.

Screening involves a preliminary cursory examination for drugs of misuse. While Screening is an economical approach to dismiss drug consumption in the majority of cases, a positive finding needs validation to be permissible in court. Any specimens that are tentatively positive in Screening must have a secondary verification.

If a specimen is provisionally positive in the Screening phase, more hair is taken from the initial sample and readied for Extraction. In this phase, drugs are removed from hair at significantly lower levels than other techniques (e.g., urine or saliva), which is why hair drug testing is the most challenging method to carry out.

Certification of any positive screening finding is accomplished through GC/MS, GC/MS/MS or LC/MS/MS. All provisional positive specimens are cleansed before confirmation as necessary. The entire lab sequence from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair categorization and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
