For EtG, additional tests, and assays exceeding 10 panels, a 150mg sample is advisable. Using a jeweler’s scale to measure the specimen is recommended. When scalp hair is absent, a comparable quantity of body hair can be utilized. When we mention head hair, it specifically refers to scalp hair, whereas body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

Accessioning, Screening, Extraction, and Confirmation are the primary steps involved in the lab processing of drug test outcomes.

Accessioning consists of inserting a sample into a lab's system for the initial processing. This step involves confirming that the sample was correctly sealed and sent, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any supplementary data required beyond what an electronic chain of custody system provides.

Screening acts as a preliminary rapid assessment for potential drug misuse. Though Screening is a cost-efficient approach to dismiss drug usage in most samples, a positive result needs confirmation for legal acceptance. All samples testing presumptively positive in Screening necessitate secondary confirmation.

Upon a presumptive positive result during the Screening stage, additional hair is extracted from the original sample to prepare for Extraction. At this point, drugs are isolated from hair at a significantly reduced concentration compared to other techniques (such as urine or oral fluid), which is why hair drug screening presents as the most challenging of methodologies.

Any confirmed positive Screening result proceeds to confirmation using GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples presumed positive undergo a wash before confirmation, as necessary. Each stage from Accessioning through to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair conformance and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
