If scalp hair isn't available, an equal volume of body hair may be collected. When "head hair" is mentioned, it refers solely to scalp hair. "Body hair" encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory system. This includes ensuring the sample was sealed and transported correctly, assigning it a unique LAN (Laboratory Accessioning Number), and performing any further data entry required that isn't covered by an electronic chain of custody system.

Screening provides a quick preliminary check for drugs of abuse. Although Screening is a cost-efficient means of excluding drug usage in the majority of cases, any positive screening must be confirmed to be valid in legal proceedings. Samples that test positive on preliminary Screening require further confirmation.

In instances where a sample tests positive during the Screening phase, more hair is extracted from the initial sample for preparation in the Extraction phase. This stage involves extracting drugs from hair at substantially lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before confirmation, all samples presumed positive are washed if necessary. The entire laboratory procedure from Accessioning to Confirmation is conducted under the oversight of the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
