What it is, How it works

When sampled near the scalp, hair can present a detection period of up to approximately 90 days for alcohol and other drugs. Hair sampling is easy to perform, relatively hard to tamper with, and simple to transport.

A 1.5-inch sample comprising roughly 200 hair strands (comparable in size to a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal quantity for screening and confirmation purposes. For tests involving EtG, add-ons, or more than 10 panels, 150mg of the sample is advisable. It is preferred to use a jeweler’s scale to weigh the sample accurately. If scalp hair is unavailable, a corresponding amount of body hair could be utilized. When talking about head hair, we specifically mean scalp hair. Body hair pertains to any hair that is not on the scalp, like facial or axillary hair.

Process Overview

The four major steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial handling of a sample within a laboratory’s framework. This involves confirming the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not furnished by an electronic chain of custody system.

Screening is an initial fast inspection for drugs of abuse. Although Screening is an economical method to exclude drug usage in the majority of samples, a positive screen necessitates confirmation to be acceptable in judicial proceedings. Any samples giving a presumptively positive result in Screening require subsequent confirmation.

Should a sample test positive in the Screening phase, additional hair is extracted from the original sample for Extraction. In this stage, drugs are obtained from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most complex method to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positive results are washed before confirmation if necessary. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
