A sample of 1.5 inches, approximately 200 strands (similar to the thickness of a #2 pencil) nearest to the scalp, provides 100mg of hair, which is ideal for both screening and confirmation. For EtG, extra components, and tests beyond 10 panels, it is recommended to collect 150mg of the sample. The specimen should be weighed with a jeweler’s scale. If head hair is not available, a comparable quantity of body hair may be used. When we mention head hair, we are specifically referring to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in laboratory examination of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample into a lab's system, which verifies that the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and accomplishes any additional data input not available via an electronic chain of custody framework.

Screening provides an initial rapid assessment for substances of abuse. While Screening is cost-efficient for eliminating drug usage in most samples, a positive screen must be validated to be admissible in legal proceedings. Any samples preliminarily positive in Screening necessitate a secondary Confirmation.

Should a sample show initial positivity in the Screening phase, additional hair is taken from the original sample and prepared for Extraction. During this phase, drugs are withdrawn from hair at a significantly lower concentration compared to other methods (ex. urine or oral fluid), making hair drug screening the most complex methodology.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples preliminarily positive undergo cleansing prior to Confirmation if needed. The full laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 norms.
