A sample measuring 1.5 inches and consisting of about 200 hair strands (resembling the thickness of a #2 pencil) from the scalp will yield 100mg of hair, which is the optimal amount for tests and confirmations. For EtG enhancements and/or tests beyond 10 panels, a specimen of 150mg is advised. Utilizing a jeweler's scale for weighing the specimen is recommended. If scalp hair isn't available, a similar quantity of body hair can be gathered. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (such as facial hair, axillary hair, etc.).

Process Overview

The comprehensive laboratory processing of a drug test outcome involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the preliminary integration of a sample into a lab's system. This phase includes ensuring the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and filling in any remaining data entry not supplied by an e-chain of custody system.

Screening entails an initial swift assessment for drug abuse. Although Screening is a cost-efficient approach for excluding drug usage in the majority of specimens, a positive screen must be confirmed for legal validity. Samples that are presumptively positive in Screening necessitate a subsequent confirmation.

Samples identified as presumptive positive in Screening undergo further extraction from the initial sample for analysis. During Extraction, drugs are retrieved from hair at lower concentrations compared to other methods (such as urine or oral fluid), thus making hair drug screening a challenging procedure to execute.

Any positive screening outcomes are corroborated through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed prior to confirmation where necessary. Each stage of the laboratory process from Accessioning through Confirmation is scrutinized under both the CAP Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
