When samples are cut from the scalp area, they can reveal drug and alcohol consumption dating back up to around three months. Collecting hair is straightforward, hard to tamper with, and easy to transport.

An ideal sample involves a 1.5-inch segment of roughly 200 hair strands (comparable in size to a #2 pencil) nearest the scalp, which provides 100mg of hair for both testing and confirmation. For assessments like EtG, additional checks, or those surpassing 10 panels, a sample weighing 150mg is suggested. We advise using a jeweler’s scale to weigh the sample. Should scalp hair be inaccessible, an equivalent portion of body hair can be used. In this context, head hair pertains solely to scalp hair, while body hair encompasses other types (e.g., facial, axillary).

Process Overview

The lab processing of a drug test result involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into a lab's system. This process includes confirming the sample's proper sealing and shipment, generating a random LAN (Laboratory Accessioning Number), and completing any missing data entry not managed by an electronic chain of custody system.

Screening involves a swift preliminary test for abused drugs. While Screening is an economical approach to eliminate drug use suspicions in most samples, a positive finding needs confirmation to be legally valid. Samples indicating a positive result in Screening require a subsequent verification.

If a sample tests presumptively positive during Screening, additional hair is taken from the initial sample and prepared for Extraction. This phase entails extracting drugs from hair at a concentration significantly lower than other methods (such as urine or oral fluid), highlighting the complexity of hair drug testing.

Confirmation of any positive screen outcome is carried out by GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed before confirmation if necessary. The entire lab process, from Accessioning to Confirmation, undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
