What it is, How it works

When taken from near the scalp, hair can reveal alcohol and drug intake spanning approximately three months. Hair collection is straightforward, challenging to tamper with, and convenient to transport.

A sample length of 1.5 inches, comprising around 200 hair strands (equivalent to the diameter of a #2 pencil) close to the scalp will yield 100mg of hair, which is optimal for screening and confirmation. For EtG, supplemental tests, or panels exceeding 10, a specimen weighing 150mg is advised. Utilizing a jeweler’s scale is suggested for weighing the sample. If scalp hair is not obtainable, a comparable quantity of body hair may be used. When mentioning head hair, we mean scalp hair exclusively. Body hair accounts for other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is primarily divided into four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a specimen into a laboratory’s database. This process confirms that the sample was suitably sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and fulfills any extra data entry absent in an electronic chain of custody framework.

Screening is a preliminary test for the presence of drugs. Although Screening is a cost-efficient method to dismiss drug usage in the majority of samples, a positive screen has to be verified to be legally acceptable. Any specimens that return presumptively positive in Screening must undergo secondary confirmation.

If a specimen tests presumptively positive during the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. Here, drugs are retrieved from hair at significantly lower concentrations compared to other methodologies (e.g., urine or saliva), making hair drug screening the most intricate method to execute.

Confirmation of any positive screening is undertaken using techniques like GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive are washed before confirmation if necessary. The entire laboratory procedure, from Accessioning through to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
