What it is, How it works

Hair offers a prolonged narrative of alcohol and drug use by securing biomarkers within the fibers of the hair strand as it grows. When obtained near the scalp, hair can deliver approximately a 3-month detection span for substances like alcohol and drugs. Hair is straightforward to collect, relatively resistant to tampering, and easy to transport.

A 1.5-inch segment of around 200 hair strands (roughly the thickness of a #2 pencil) near the scalp will yield 100mg of hair, which is an optimal sample size for initial tests and confirmation. For EtG, additional tests, and tests beyond 10 panels, 150mg of the specimen is advised. We suggest using a jeweler’s scale for accuracy. If scalp hair isn’t present, a similar volume of body hair is acceptable. By head hair, we mean scalp hair exclusively. Body hair encompasses all other types such as facial or axillary hair.

Process Overview

The laboratory handling of drug test results primarily involves Accessioning, Screening, Extraction, and Confirmation.

Accessioning details the initial sample processing within a lab system. Tasks include verifying proper sample sealing and shipment, assigning a LAN (Laboratory Accessioning Number) randomly, and completing any outstanding data entry not covered by an electronic chain of custody system.

Screening offers a preliminary assessment for drugs of abuse. Though Screening is cost-efficient for eliminating drug use on most samples, a positive result must be confirmed for legal acceptance. For samples indicative of drug presence during Screening, a secondary confirmation is mandatory.

If Screening suggests a positive result, additional hair is extracted from the original sample for the Extraction stage. This stage involves obtaining drugs from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), hence making hair drug screening the most complex method.

Confirmation for any positive screening outcome employs techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all samples presumed positive undergo washing if necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
