It is easy to collect, relatively resistant to tampering, and convenient to transport.

A 1.5-inch section of roughly 200 hair strands (approximately the diameter of a #2 pencil) adjacent to the scalp yields 100mg of hair, serving as the optimal quantity for screening and verification. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advisable. We suggest measuring the sample with a jeweler's scale. If scalp hair is unavailable, a similar amount of body hair may be gathered. When discussing head hair, we refer specifically to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial integration of a sample into a laboratory's system. This step involves confirming the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Although Screening is a cost-effective method to eliminate drug use in most samples, a positive screen must be confirmed for legal admissibility. Samples yielding a presumptive positive during Screening necessitate a follow-up confirmation.

Should a sample test presumptively positive at the Screening phase, extra hair is extracted from the initial specimen for further processing in Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to conduct.

Confirmation of any positive screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples undergo washing as needed. The comprehensive laboratory procedure from Accessioning to Confirmation is evaluated in accordance with the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
