Process Overview
The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.
During Accessioning, the initial processing of a sample within a laboratory’s system takes place. This step includes confirming the sample’s proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.
Screening is a preliminary quick assessment for drug abuse detection. Although Screening is a cost-effective method for excluding drug use in the majority of cases, a positive result requires validation to be legally admissible. Samples presumptively positive in Screening necessitate a secondary confirmation.
In instances where a sample tests presumptively positive during Screening, additional hair is extracted from the original sample and prepared for the Extraction stage. Here, drugs are extracted from hair at concentrations significantly lower than other approaches (e.g., urine or oral fluid), making hair drug screening the most challenging method to conduct.
Positive screening results are ultimately confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation when necessary. The entire laboratory process, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation of ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
