What it is, How it works

Hair analysis has become recognized as a formidable instrument for detecting drug and alcohol consumption. Hair strands capture a long-term record of alcohol and drugs through embedded biomarkers in the fiber of the hair as it grows. When retrieved close to the scalp, it can reveal up to a three-month span of exposure to alcohol and drugs. Hair is straightforward to gather, relatively hard to tamper with, and convenient to transport.

A 1.5-inch portion of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample for both screening and verification. For EtG, additional considerations, and/or tests exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale for weighing. If scalp hair is absent, an equal quantity of body hair can be collected. By head hair, we mean scalp hair exclusively. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The four key phases in the lab analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample into a laboratory's workflow. This entails ensuring the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and fulfilling any extra data entry not covered by an electronic chain of custody system.

Screening is a primary rapid assessment for substances of abuse. Although Screening is a cost-saving method to exclude drug usage in most samples, a positive screen necessitates confirmation for legal validity. Samples deemed presumptively positive in Screening require further confirmation.

If a sample appears presumptively positive during Screening, additional hair is retrieved from the original sample for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other methods like urine or oral fluid, making hair drug screening notably challenging.

Positive screening outcomes undergo Confirmation using techniques like GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are washed prior to confirmation if necessary. The comprehensive lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 criteria.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
