What it is, How it works

Hair analysis is recognized as a potent method for identifying drug and alcohol consumption. Hair offers an extended timeline of alcohol and drug activity by embedding biomarkers within the hair strand fibers as they grow. When collected near the scalp, hair offers about a 3-month detection span for alcohol and drugs. Collecting hair is straightforward, challenging to tamper with, and easy to transport.

A 1.5-inch segment of roughly 200 hair strands (about the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal amount for both initial screening and confirmation. For tests involving EtG, add-ons, or more than 10 panels, 150mg is advised. We suggest measuring the sample with a jeweler’s scale. If scalp hair is not available, the same amount of body hair can be collected. When we mention head hair, it is specifically about scalp hair. Body hair refers to all other hair types (facial, axillary, etc.).

Process Overview

The four key stages in the lab processing of drug test results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample into a lab’s database. This step includes confirming that the sample was sealed and shipped correctly, issuing a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry absent from an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. While Screening is a cost-efficient method to eliminate drug usage for most samples, a positive screen must be confirmed for legal admissibility. Samples deemed positive in Screening require further confirmation.

If a sample is deemed positive during the Screening stage, additional hair is taken from the original specimen for preparation for Extraction. Here, drugs are removed from hair at far lower concentrations than in other techniques (e.g., urine or oral fluid), making hair drug screening the most intricate method to execute.

Confirmation of any positive screening outcome uses GC/MS, GC/MS/MS or LC/MS/MS. All samples that test positive initially are washed before confirmation as necessary. The full lab process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
