A 1.5-inch sample of approximately 200 hair strands (about the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. We suggest weighing the specimen using a jeweler's scale. If scalp hair isn't available, an equivalent quantity of body hair may be gathered. By head hair, we specifically mean scalp hair only. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a laboratory's system, including verification of proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry not fulfilled by an electronic chain of custody system.

Screening is an initial swift examination for drugs of abuse. While a cost-efficient approach to rule out drug usage in most samples, a positive screen demands confirmation for court admissibility. Samples that test presumptively positive in Screening necessitate a secondary confirmation.

In case of a presumptive positive during the Screening phase, extra hair is withdrawn from the existing specimen and prepared for Extraction. In Extraction, drugs are removed from hair at significantly lower concentrations compared to other methodologies (e.g., urine or oral fluid), marking hair drug screening as the most complex methodology to execute.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The comprehensive laboratory process from Accessioning to Confirmation reviews under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
