For EtG, additional tests, and those exceeding 10 panels, 150mg of the sample is advised. Measuring the sample with a jeweler’s scale is recommended. If head hair cannot be obtained, an equivalent amount of body hair is an alternative. Referring to head hair implies scalp hair; whereas, body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of drug test results involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where samples are integrated into the laboratory system. This involves checking that samples were sealed and shipped accurately, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any other necessary data recording absent from an electronic chain of custody system.

Screening is a preliminary rapid evaluation for substance abuse. Although Screening is a cost-efficient method to eliminate most samples from further testing, a positive screen must be confirmed for legal admissibility. Samples testing presumptively positive in Screening necessitate a secondary confirmation.

For any samples flagged as presumptively positive during Screening, additional hair is extracted from the original sample and readied for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than those in other methods (such as urine or oral fluid), highlighting the complexity of hair drug testing.

Positive screening results are confirmed through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo a washing process before confirmation. The entire laboratory procedure, from Accessioning to Confirmation, is conducted under the scrutiny of both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
