Process Overview

The drug testing laboratory process encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

This process entails confirming proper sealing and shipment of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody.

Upon a presumptive positive result in the Screening phase, additional hair is extracted from the original sample for Extraction. In this process, drugs are extracted from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), highlighting the complexity of hair drug testing.

Confirmation of positive Screening results is executed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. The full laboratory procedure from Accessioning to Confirmation is subject to review according to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.
