What it is, How it works

A sample of 1.5 inches consisting of approximately 200 hair strands (comparable to the thickness of a #2 pencil) proximate to the scalp will yield 100mg of hair, which is the optimal quantity for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg specimen is advisable. We suggest using a jeweler’s scale for measuring the specimen. If scalp hair isn't accessible, a comparable quantity of body hair is an alternative. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the introductory handling of a sample within a lab’s framework. This process verifies that the sample has been properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and fulfills any additional data entry that isn't part of an electronic chain of custody system.

Screening executes an initial rapid assessment for substance abuse. Although Screening is an economical approach to exclude drug use in most samples, a positive screening necessitates confirmation for legal admissibility. Samples demonstrating presumptive positive results in Screening mandate secondary confirmation.

When a sample yields a presumptive positive result in the Screening phase, more hair is extracted from the original specimen and prepared for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), which is why hair drug testing is recognized as the most challenging methodology to conduct.

Confirmation of any positive result from Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive outcomes undergo washing before confirmation if necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized both under the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
