When sourced near the scalp, hair can offer up to nearly a 90-day detection period for alcohol and other drugs. Hair is easy to gather, somewhat challenging to tamper with, and straightforward to transport.

A 1.5-inch sample consisting of about 200 hair strands (equivalent to a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal quantity for testing and verification. For EtG, additional analyses, and/or testing beyond 10 panels, 150mg of the sample is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is unattainable, a similar amount of body hair can be obtained. When mentioning head hair, we specifically mean scalp hair. Body hair includes all other hair categories (facial, underarm, etc.).

Process Overview

The four primary stages in the laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample into a laboratory’s framework. This comprises checking that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entry absent from an electronic chain-of-custody system.

Screening constitutes an initial rapid examination for drugs of abuse. While Screening is a cost-effective method to exclude drug usage in the majority of samples, a positive screen must be validated to be legally acceptable. Any samples that are assumed positive in Screening indeed necessitate a secondary confirmation.

If a sample is assumed positive in the Screening phase, additional hair is extracted from the initial sample and set for Extraction. At this stage, drugs are extracted from hair at a significantly lower concentration than in other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging technique to perform.

Confirmation of any positive Screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All assumed positive samples are cleansed before confirmation as required. The entire laboratory sequence from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
