A 1.5-inch segment of around 200 hair strands (roughly the girth of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample size for analysis and verification. For EtG, supplementary tests, and/or panels exceeding 10, a 150mg quantity is advised. We recommend using a jeweler’s scale for weighing. If scalp hair is not available, a comparable quantity of body hair can be collected. By head hair, we specifically refer to scalp hair. Body hair encompasses all other hair types (facial, axillary, and so on).

Process Overview

The four primary steps in laboratory processing for a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a sample into a lab’s system. This step ensures the sample was properly sealed and shipped, involves assigning a random LAN (Laboratory Accessioning Number), and finalizes any remaining data entry not included in an electronic chain of custody system.

Screening entails an initial rapid assessment for substances. While Screening provides a cost-efficient method to dismiss drug use in most samples, a verified result is necessary for legal proceedings. Samples presumed positive in Screening require additional confirmation.

If Screening indicates a presumptive positive, extra hair is taken from the initial sample for Extraction. In Extraction, substances are retrieved from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), explaining why hair drug analysis is the most challenging method to conduct.

Confirmation of any positive results from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation when necessary. The entire lab process from Accessioning through Confirmation is assessed under both CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
