What it is, How it works

Hair analysis has been recognized as an effective method for identifying substance use. Hair can hold a historical record of alcohol and drugs by capturing biomarkers within the strands as hair grows. When clipped close to the scalp, hair offers a timeframe of up to about 3 months for detecting alcohol and drugs. Hair collection is straightforward, hard to tamper with, and convenient for shipping.

A sample measuring 1.5 inches containing approximately 200 hair strands (roughly the thickness of a #2 pencil) from near the scalp provides 100mg of hair, which is ideal for testing. For EtG, additional tests, or panels over 10, 150mg of the specimen is suggested. We advise weighing the sample with a jeweler’s scale. If scalp hair isn’t accessible, you can use a similar quantity of body hair. Head hair specifically refers to scalp hair, whereas body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary stages in processing a drug test in a laboratory setting include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into the lab’s database. This process verifies that the sample is properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry missing from an electronic chain of custody.

Screening is a preliminary, rapid examination for drugs of abuse. While it is a cost-effective approach for excluding drug use in most samples, any positive findings need affirmation to be legally valid. Presumptively positive samples in Screening require further confirmation.

In cases where a sample tests presumptively positive during Screening, additional hair from the original specimen is prepared for Extraction. At this stage, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine, oral fluid), which is why hair drug testing is the most complex method to carry out.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed if necessary before confirmation. The complete lab process from Accessioning to Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
