Body hair includes all other hair types (e.g., facial, axillary, etc.).

Process Overview

There are four primary steps in processing a drug test result in the laboratory: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where a sample is entered into a laboratory's system. This involves checking that the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not included in an electronic chain of custody system.

Screening provides an initial rapid evaluation for drugs of abuse. Although Screening is an economical method to eliminate drug use in most samples, a positive screen must be confirmed for it to be admissible in court. Samples that test presumptively positive during Screening require secondary confirmation.

If Screening indicates a presumptive positive result, additional hair from the original specimen is used and prepared for Extraction. During this phase, drugs are isolated from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), which makes hair drug screening more challenging to execute.

Confirmation of any positive result from screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that show presumptive positivity are washed prior to confirmation as necessary. The entire laboratory process, from Accessioning to Confirmation, is assessed under the CAP (College of American Pathologists) Hair designation and is accredited to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
