A 1.5-inch sample comprising around 200 hair strands (roughly equivalent to the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is advised. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair cannot be collected, an equivalent amount of body hair may be used. “Head hair” specifically means scalp hair. “Body hair” encompasses all other hair types (facial, axillary, etc.).

Process Overview

The processing of a drug test result in the laboratory consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory step where a sample enters the laboratory system. This phase includes verifying that the sample was correctly sealed and shipped, assigning a LAN (Laboratory Accessioning Number), and doing any further data entry not covered by an electronic chain of custody system.

Screening involves an initial rapid test for drugs of abuse. While Screening is an economical way to exclude drug use in the majority of samples, a positive result from a screen needs confirmation to be legally valid. Any samples that screen presumptively positive require additional verification.

If a sample yields a presumptive positive result in Screening, additional hair is removed from the initial specimen and prepared for Extraction. In this stage, drugs are extracted from hair at a concentration significantly lower than in other testing modalities (e.g., urine or oral fluid), making hair drug tests the most complex to execute.

The Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
