A sample measuring 1.5 inches with roughly 200 hair strands (similar to the thickness of a #2 pencil) from near the scalp provides 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, it's advisable to have 150mg of hair. Using a jeweler’s scale for weighing the sample is suggested. If scalp hair can’t be sourced, a similar quantity of body hair can be used. When discussing head hair, it refers strictly to scalp hair. Body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

In drug test processing, the four primary steps executed in the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of the sample into the lab’s system. Steps here verify proper sealing and shipping of the sample, assign a random LAN (Laboratory Accessioning Number), and complete any data entry missing in an electronic chain of custody.

The Screening phase provides a preliminary analysis for substances of abuse. While being a budget-friendly method for eliminating drug usage in most cases, a positive screen calls for verification to hold up in court. Presumptively positive samples in Screening are obligated to undergo a secondary validation.

For samples indicated as presumptively positive in the Screening phase, more hair is drawn from the initial batch for the Extraction process. This phase extracts drugs from hair at notably lower concentrations than other techniques (e.g., urine or oral fluid), which renders hair drug testing more challenging.

Confirmation of positive Screening outcomes employs GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are cleansed before confirmation if necessary. The complete laboratory sequence, from Accessioning through Confirmation, is reviewed adhering to both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
