For EtG, additional tests, or tests exceeding 10 panels, a sample size of 150mg is preferable. It's suggested to weigh the sample using a jeweler's scale. In cases where scalp hair isn't available, a comparable quantity of body hair may be used. Reference to head hair involves scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The procedure utilized by labs to process a drug test includes: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves incorporating a sample into the lab's system. This step includes ensuring proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any remaining data entry not covered by an electronic chain of custody system.

Screening is a preliminary fast check for controlled substances. Although it is an economical method to exclude drug use on most samples, a positive result necessitates confirmation for legal admissibility. Any samples hinted to be positive in Screening necessitate a secondary verification.

Should a sample show a presumptive positive in Screening, additional hair is extracted from the initial sample for further preparation in the Extraction phase. Positive Screening results are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All preliminarily positive samples undergo washing before confirmation as required. The entire lab processing from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
