Hair is straightforward to collect, somewhat challenging to adulterate, and convenient to transport.

A sample measuring 1.5 inches with roughly 200 hair strands (similar to the diameter of a #2 pencil) near the scalp will yield 100mg of hair, which is the ideal sample size for screening and confirmation. For EtG, add-ons, and/or tests over 10 panels, 150mg of the specimen is advised. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair is not available, an equal quantity of body hair can be used. When referring to head hair, it denotes scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is entered into a laboratory's system. This step ensures the sample was properly sealed and shipped, involves assigning a random LAN (Laboratory Accessioning Number), and completes any additional data entry not managed by an electronic chain of custody system.

Screening performs an initial rapid assessment for substance abuse. While Screening is a cost-efficient method to exclude drug usage in most samples, a positive result requires confirmation to be acceptable in legal proceedings. Any samples with a presumptive positive result in Screening must undergo secondary confirmation.

When a sample tests presumptively positive during Screening, additional hair is extracted from the initial specimen for the Extraction phase. Here, drugs are released from hair at a much lower concentration compared to other methodologies (e.g., urine or oral fluid), making hair drug screening a challenging method to execute.

Any positive screening result is verified through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The full laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.
