Recently, drug testing methodologies have increasingly incorporated hair follicle tests. Numerous employers, judicial entities, and substance abuse specialists now prefer hair drug testing over traditional urine analysis. Hair follicle tests are favored by organizations with strict no-drug policies, legal institutions, and probation services. Hair analysis is increasingly recognized as an invaluable method for uncovering drug and alcohol consumption. Hair can capture a prolonged account of alcohol and drug presence by embedding biomarkers within the fibers of the advancing hair strand. When excised near the scalp, hair can reveal a detection period of up to roughly 3 months for alcohol and drugs. Hair is straightforward to collect, challenging to adulterate, and straightforward to transport.
A 1.5-inch portion of around 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp will offer 100mg of hair, which is the optimal sample for analysis and validation. For EtG, additional checks, and/or examinations exceeding 10 panels, 150mg of the specimen is advised. We advocate for measuring the sample on a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be obtained. When mentioning head hair, we mean scalp hair solely. Body hair is all other hair categories (facial, axillary, etc.).
Procedure Overview
The procedure for processing a drug test result in the lab involves four key steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning pertains to the initial integration of a sample into a laboratory’s framework. This consists of confirming the sample was securely sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any remaining data entry not covered by an electronic custody system.
Screening involves a rapid assessment for substances of abuse. While Screening offers a budget-friendly means to rule out drug consumption for most samples, a positive test needs confirmation for legal validity. Any samples exhibiting a presumptive positive during Screening necessitate further validation.
Should a sample indicate a presumptive positive at the Screening phase, additional hair is extracted from the original sample and readied for Extraction. During this phase, substances are drawn from hair to a far lower concentration compared to other methods (e.g., urine or oral fluid), which contributes to hair drug testing being the most intricate method to perform.
Verification of any presumptive positive result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a presumptive positive outcome undergo cleansing prior to final confirmation if required. The complete laboratory method from Accessioning to Confirmation is assessed under CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

Note: Often referred to as "hair follicle tests", these tests examine the hair strand rather than the follicle beneath the scalp.

The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.
Biomarkers and Pharmacokinetics: The identification and validation of reliable biomarkers and pharmacokinetic studies are fundamental aspects of drug testing and analysis. The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.
Forensic Applications and Interpretation: One of the vital objectives of hair testing is its application in forensic investigations and legal proceedings. The SoHT meeting in Verona emphasized the significance of rigorous interpretation of analytical results and the need for standardized protocols. Manuscripts in this section present advancements in forensic toxicology, and have explored the challenges posed by emerging designer drugs and strategies to combat their proliferation. The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The special issue presents also manuscripts that delve into the ethical implications of drug testing in drug facilitated sexual assaults.
Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed, and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
