For optimal screening and affirmation, a hair sample of 1.5 inches, consisting of about 200 strands (comparable to a #2 pencil), yields 100mg, which is ideal. For EtG and additional tests beyond 10 panels, 150mg is suggested. We advocate for specimens to be weighed using a jeweler's scale. If scalp hair isn’t available, a similar quantity of body hair may be used. When referencing head hair, we only mean scalp hair, while body hair encompasses all other types like facial or axillary hair.

Process Overview

Four principal phases form the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is integrated into a lab’s system, involving verification of proper sealing and shipping, assignment of a unique LAN (Laboratory Accessioning Number), and completion of any additional data input not covered by an electronic chain-of-custody system.

Screening is a preliminary evaluation for drug traces. It’s cost-efficient for ruling out drug use in most samples, but positives must be verified for legal validity. Samples with presumptive positives in Screening necessitate secondary confirmation.

In cases of presumptive positivity in Screening, more hair is extracted from the source sample for Extraction, where drugs are isolated at significantly lower concentrations than in other methods like urine or oral fluid, underscoring the complexity of hair drug screening.

Confirmation of positive screen results is performed via GC/MS, GC/MS/MS, or LC/MS/MS, with all presumptive positive samples undergoing cleansing as required. The entire lab process, from Accessioning to Confirmation, aligns with both CAP (College of American Pathologists) Hair designation and the standards of ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
