A 1.5-inch section of around 200 hair strands (resembles the size of a #2 pencil) next to the scalp provides 100mg of hair, which is the optimal quantity for testing and confirmation. For EtG, additional tests, or tests exceeding 10 panels, it is advised to use 150mg of the sample. Utilizing a jeweler’s scale to measure the specimen is suggested. Should scalp hair be unavailable, an equivalent quantity of body hair may be used. "Head hair" specifically means scalp hair, while "body hair" includes other hair types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial intake of a sample into a lab’s system, ensuring the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), plus completing any additional documentation not covered by an electronic chain of custody platform.

Screening entails an initial fast check for harmful substances. While Screening economically rules out drug usage in the majority of samples, a confirmation is necessary for any court-admissible positive screen. Presumptive positive samples from Screening must undergo a subsequent confirmation.

Should a specimen be presumptively positive during Screening, more hair is extracted from the primary sample for the Extraction stage. Drugs are extracted from hair at concentrations lower than other methods (e.g., urine or oral fluid), rendering hair drug screening the most intricate methodology to conduct.

Confirmation of positive screening findings is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The full lab process from Accessioning to Confirmation is inspected according to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
